NEW PASSPORT APPLICATION

1. Fill out the application form

2. Attach original documents and copies of 1 (one) sheet:

a. Valid Identity Card (KTP) or certificate of transfer abroad

b. Family Card (KK), contains the same address as the KTP

c. Birth certificate / marriage certificate or marriage book / diploma / baptism letter (document containing name, place of birth, date of birth, and parent's name);

d. Indonesian citizenship letter for foreigners who obtain Indonesian citizenship through citizenship or submission of statements to choose citizenship in accordance with statutory provisions

e. Name change letter from the authorized official for those who have changed names.

A passport is a state document, so the holder is asked to be careful in storing and keeping it, so that it is not lost and falls into the hands of the unauthorized.

The requirements for replacing the lost passport are outlined as follows:

Make a report of losing your passport to the Republic of Indonesia Police Office or the authorities if you are abroad.
If the passport is lost, it is requested to immediately report to the Immigration Office where the holder is domiciled to request reimbursement.
The replacement of a lost / damaged passport is carried out after the Examination Report (BAP), and gets the approval of the Head of the Regional Office of the Ministry of Law and Human Rights in this case the Head of the Immigration Division.
If the inspection found elements of carelessness or negligence, accompanied by unacceptable reasons, giving a passport can be suspended for 6 (six) months up to a maximum of 2 (two) years.
If the passport is lost / damaged overseas, report immediately to the Indonesian Representative abroad.
